What is a Schedule 10 EI and PPIP Premiums in Canada form?

If you're from Quebec, then the schedule 10 is used to calculate the premiums you pay on employment or self-employment income for a Provincial Parental Insurance Plan (PPIP). It's also used to see if you contributed too much towards Employment Insurance (EI) indicated on your T4 tax forms or RL-1s. To claim the amount you must complete this schedule to see if you reported only a net self-employment or employment income (inside and outside of Canada) for $2000 or more or if one of your T4 slips is for a province of employment other than Quebec with an amount of $2000 or more.
